ABQAURP sub-specialty credits: Transitions of Care 1.0, Patient Safety/Risk Management 1.0, Physician Advisor 1.0, & Case Management 1.0

Despite significant treatment advances, new evidence and recently updated guidelines, high rates of hospital readmissions and poor outcomes persist for patients with heart failure. Experts recognize the remarkable potential to reduce unnecessary hospitalizations and improve patient outcomes through effective interventions at critical transition points to bridge gaps in care.

Developed in partnership with the National Transitions of Care Coalition (NTOCC) and led by a multidisciplinary advisory board, this Heart Failure Transition of Care (TOC) pathway provides essential interventions and tools to support system leaders and the interprofessional care team - including patients and their caregivers, incorporating the latest evidence-based treatment and management approaches to ensure effective and safe transitions of care for patients with heart failure across health care settings.

At the conclusion of this activity, attendees should be able to:
  • Relate to gaps and barriers to safe transitions of care for heart failure patients.
  • Assess the impact that social determinants of health have on adherence and readmissions.
  • Discuss seven key elements for improving transitions and care coordination for patients and their family caregivers.
  • Evaluate the importance of doing a quality improvement study on improving transitions for patient populations with heart failure.
