Transitions of Care Certification
Introducing the First International
Transitions of Care Certification
Move Quality Care Forward!
Certification validates your advanced skills and expertise to manage and
lead in today’s complex health care environment
The American Board of Quality Assurance and Utilization Review
Physicians (ABQAURP) has added Transitions of Care to its Health Care
Quality and Management (HCQM) Certification.
In an era of accountability, HCQM board certification demonstrates that
Diplomates have the practical knowledge and the tools that can reduce
medical errors, ensure patient safety, eliminate wasteful spending and
unnecessary services, while avoiding potentially harmful delays in care.
Certification demonstrates to internal and external entities that you,
your group practice, or hospital is accountable. The result is a safer,
more efficient, and cost-effective system that better satisfies the
needs of patients, providers, purchasers, and payers. The Transitions
of Care Sub-Specialty has been incorporated as a vital component of the
Health Care Quality and Management board certification.
As we accept the challenge to develop a health care delivery system that
advances the quality and the efficiency of health care in a manner that
is cost-efficient, we face not only tremendous opportunities, but also a
great urgency for broad transformation.
The delivery of health care services is sometimes disorganized and
haphazard with patients shuffled from one environment or provider of
care to another with little advocacy, no established transitional care
plan, and absolutely no idea that it should not be that way. Health care
consumers frequently move from door to door; episode of care to episode
of care without a champion to coordinate that care. This often prompts a
downward trajectory of their health status that can not only prompt
readmissions to an acute care facility, but also cause physical,
emotional, and financial compromise that may negatively impact the
quality of the patient’s life.
Certification in HCQM with sub-specialty accreditation in Transitions of
Care puts you on the leading edge in today’s competitive job market.
