From our knowledge of Social Determinants of Health (SDoH), we understand that direct medical care only influences 20% of an individual’s overall health, while 80% is influenced by factors such as access to health care, gender, culture, income, social support networks, education and employment status, physician and social environments, personal health practices and coping skills, among others. Health care providers are left with a limited window to engage, educate, and partner with patients in their health care.
Consequently, the emphasis the past few decades has been on learning more about disparities in our communities and bridging those gaps through technology, innovative and “disruptive” models of health care delivery, and service excellence. All while continuing to aim for the delivery of the highest quality, cost-effective health care possible.
Attend the conference to develop strategies to integrate technology, disruptive delivery models, and service excellence into practice to address the challenges of social determinants of health, improve the population health of communities served, and expand opportunities and patient outcomes for providers.
