Spotlight Course: Patient Centered Care within the Value-Based Care Model
Douglas Keith Fernandez, MD
Chief Clinical Officer
Privia Health
The increasing complexity of the delivery of healthcare has raised the risk of healthcare becoming less personal. Challenges of time constraints, lack of tools to prioritize our highest risk patients, and reimbursement models that are not always consistent with practice preferences are making it difficult for physicians to drive optimal healthcare through traditional patient centered models of care. Despite support from clinical extenders, patients feel that the provider-patient relationship is becoming a thing of the past. This is further exacerbated by the stresses of burnout, and the enhanced emphasis on the business aspects for medicine which at times create barriers for physicians to enjoy the practice of medicine. In this course, hear from leadership of a physician-led practice describing how the physician-patient relationship is emphasized and supported through tools, technology, and innovative value-based care delivery models.

Touchpoints and Nurse Sensitive Indicators: Exploring Opportunities to Identify Nursing’s Contribution to Quality in Primary Care
April 23, 2024: As patients and healthcare professionals, we all can appreciate the roles that nurses fulfill in supporting clinical systems. Patient outreach and teaching, assisting providers, overseeing quality audits, conducting telephonic and virtual triage, and responding to patients’ urgent care needs, are just a few of the many ways in which the primary care environment is enriched through nursing practice. The roles of Clinical Care Coordination and Transitional Care Coordination, both of which have been identified as important navigational elements in healthcare and which have become more developed in the past ten years, are considered to be best performed by registered nurses in primary care settings. (American Nurses Association: The Value of Nursing Care Coordination: A White Paper, 6/12: p. 6.)

February 4, 2022: ABQAURP has joined with other health care organizations in signing a letter to the National Telecommunications and Information Administration (NTIA) in support of using connected health technologies to improve patient outcomes and reducing health care costs. The letter is in support of the implementation of the Infrastructure Investment and Jobs Act and funding of its related programs. Consistently growing evidence suggests that digital health technologies improve patient outcomes, reduce hospitalizations, avoid complications, improve patient engagement, and reduce costs. From ABQAURP’s CME Chair:
May 18, 2021: ABQAURP as an organization was able to achieve a significant milestone on Friday, April 30, 2021. In view of the pandemic, like many other organizations, we were faced with the dilemma of how to accomplish our goal to hold our annual meeting safely and effectively. Although unable to do so in 2020, we eventually were able to host the 43rd Annual Health Care Quality and Patient Safety Conference, 20/20 Insight into Value-Based, Quality Care. We believe the presentations and related discussions supported our mission to improve the overall quality of health care while providing valuable information to our members and the health care community.
