Aging Life Care™ Support, also known as geriatric care management, is a holistic, client-centered approach to caring for older adults or others facing ongoing health challenges. This session will help clinicians understand the role an Aging Life Care Professional® brings to the collaborative care team and how integrating their services with community resources in transition planning can improve outcomes. Speakers will discuss the role of the care manager in assessment and monitoring, planning and problem-solving, and family/caregiver coaching.

At the conclusion of this activity, attendees should be able to:
  • Identify the role and scope of practice of an Aging Life Care Professional® (ALCP).
  • Explain the eight core areas of expertise of an ALCP, the value to the client and family in guiding an individualized medical and life care plan that addresses not only their immediate needs/problems, but supports a long-term plan of care that incorporates the entire client system, inclusive of professionals.
  • Evaluate the interactions between the care manager and the collaborative care team.
