Case managers across the care continuum have been (or will soon be) challenged by the Opioid Crisis, Shared decision-making initiatives, and Long-Term Services and Supports in the care of their patients. Case managers, especially those working in the ED setting are facing a rising rate of opioid-related visits. Effectively addressing opioid use issues are beyond being a single company’s success story. This public health travesty mandates outcomes sharing across the continuum and outside of the realm of healthcare.

Shared-Decision Making involves taking the time to assess each patient’s understanding about their health conditions and treatment options. Case managers delve into literacy issues as well as personal, cultural and linguistic preferences. It is this level of detail that informs the case management plan of care which takes into consideration each person’s ability to participate in healthcare decision-making.

Long-Term Services and Supports (LTSS) are critical links to safe home and community transitions of care. However, LTSS providers are not routinely included in the healthcare team construct, especially as pertains to care transitions from inpatient facilities to “home”.

This online course will provide insight related to the critical issues case managers face with Opioid Abuse, Shared-Decision Making and Long-Term Services and Supports..

This session was recorded live at our 2019 Annual Health Care Quality & Patient Safety Conference and is a portion of the online course entitled: Connecting People, Purpose & Passion. They may be taken individually to earn incremental CME/CE credits. However, if you plan on taking the full 8-hour course, you will not be able to claim credit for these individual sessions.

At the conclusion of the activity, attendees will be able to:

  • Demonstrate how shared decision-making initiatives impact case management
  • Discuss how Long-Term Services and Supports are changing case management practices

This course is intended for Physicians of all specialties, Nurses, and Other Health Care Professionals with appropriate CME credit and nursing contact hours offered for each profession.
